Serdolect Indications
Schizophrenia.
According to the stipulation concerning cardio-vascular safety, sertindole is prescribed only to patients for whom treatment with at least one other antipsychotics accompanied by reactions of intolerance.
Sertindole should not be used in emergency situations for the rapid relief of symptoms of exacerbation in patients.
Contraindications
Hypersensitivity to sertindole or any component of the drug.
Sertindole is contraindicated in patients with established nekoryhovanoyu hypokalemia or hypomagnesemia.
Sertindole is contraindicated in patients with clinically significant cardiovascular disease, congestive heart failure, kardiohipertrofiyeyu, arrhythmia, or bradycardia (<50 beats / min) in history, as well as patients with a syndrome of hereditary long QT or a family history of the disease, or in patients with acquisitions prolonged QT interval (450 msec QTs than in males and 470 msec in females).
Sertindole is contraindicated in patients with severe hepatic impairment.
Sertindole is contraindicated in patients receiving means to significantly prolong the QT interval. These classes include:
- Antiarrhythmics class IA and III (eg quinidine, amiodarone, sotalol, dofetilide);
- Some antipsychotics (eg, thioridazine)
- Some macrolides (eg, erythromycin);
- Some antihistamines (eg terfenadine, astemizole);
- Some quinolone antibiotics (eg gatifloxacin, moxifloxacin).
Dosage & Administration
Sertindole administered orally 1 time per day regardless of the meal.
Patients who need sedation while benzodiazepine can be assigned.
It is necessary to monitor the ECG at the beginning and during treatment with sertindole. Clinical trials have shown greater ability sertindole prolong QT interval compared to some other antipsychotic agents. The target must fully meet the required safety warnings.
Titration dose
Treatment of patients should be started on sertindole 4 mg doses a day. The dose is increased to 4 mg every 4-5 days until you reached the optimal daily maintenance dose within 12-20 mg. Thanks to the activity of sertindole as a1-blocker, at the beginning of tytratsiyi doses may experience symptoms of postural hypotension. The initial dose of 8 mg or a rapid increase in the dose significantly increase the risk of postural hypotension.
Maintenance dose
Depending on the effect, individually dose may be increased to 20 mg per day. Only in exceptional cases can be a maximum dose - 24 mg, as in clinical trials were found more effective doses above 20 mg and QT interval prolongation may be increased at the higher dose range.
