Acute and chronic inflammatory diseases of the joints (rheumatoid arthritis, juvenile arthritis, ankylosing spondylitis, gouty and psoriatic arthritis, polyarthritis, humeropathy periarthritis, osteoarthritis (including posttraumatic), Still's syndrome in adults, bursitis, nonspecific tendosynovitis, synovitis, epicondylitis) ; Bronchial asthma, asthmatic status. Interstitial lung diseases (acute alveolitis, pulmonary fibrosis, sarcoidosis of II-III degree); lung cancer (in combination with cytostatics), berylliosis, aspiration pneumonia (in combination with specific therapy), Leffler's eosinophilic pneumonia, tuberculosis (pulmonary tuberculosis, tuberculous meningitis) - in combination with specific therapy. Hepatitis. Hypoglycemic conditions. Autoimmune diseases (including acute glomerulonephritis). Nephrotic syndrome. Inflammatory diseases of the digestive tract (ulcerative colitis, Crohn's disease, local enteritis). Diseases of blood and hemopoiesis (agranulocytosis, panmyelopathy, myeloma, acute lympho- and myeloid leukemia, lymphogranulomatosis, thrombocytopenic purpura, secondary thrombocytopenia in adults, autoimmune hemolytic anemia, erythroblastopenia, congenital erythroid hypoplastic anemia). Autoimmune and other skin diseases (eczema, seborrheic dermatitis, psoriasis, Lyell's syndrome, bullous herpetiform dermatitis, pemphigus, exfoliative dermatitis). Edema of the brain postoperative, postradiation, posttraumatic, brain tumors (used after parenteral glucocorticosteroids). Diseases of the eyes, incl. allergic and autoimmune (sympathetic ophthalmia, allergic forms of conjunctivitis, allergic corneal ulcer, non-purulent keratitis, iridocyclitis, iritis, severe sluggish anterior and posterior uveitis, choroiditis, optic neuritis). Prevention of graft rejection reactions. Hypercalcemia on the background of cancer. For prevention and elimination of nausea, vomiting in cytostatic therapy.
Contradictions
Hypersensitivity to the components of the drug.
Parasitic and infectious diseases of a viral, fungal or bacterial nature, existing or recently transferred: herpes simplex, herpes zoster (viremic phase), chicken pox, measles; amebiasis, strongyloidiasis (established or suspected); systemic mycosis; active and latent tuberculosis.
Post-vaccination period (duration of 10 weeks: 8 weeks before and 2 weeks after vaccination), lymphadenitis after BCG vaccination.
Immunodeficiency conditions caused by HIV infection.
Diseases of the gastrointestinal tract: peptic ulcer of stomach and duodenum, esophagitis, gastritis, acute or latent peptic ulcer, newly created intestinal anastomosis, ulcerative colitis with perforation or abscessing, diverticulitis.
Diseases of the cardiovascular system: recently transferred myocardial infarction, decompensated chronic heart failure, arterial hypertension, propensity to thromboembolic disease.
Diseases of the endocrine system: diabetes mellitus and a violation of carbohydrate tolerance, thyrotoxicosis, hypothyroidism, Itenko-Cushing's disease.
Severe chronic renal and / or hepatic insufficiency, nephrourolythiasis. Hypoalbuminemia. Systemic osteoporosis. Myasthenia gravis. Acute psychosis. Obesity (III-IV century). Poliomyelitis (except for the form of bulbar encephalitis). Open and closed angle glaucoma.
Dosage and Administration
Dosage is set individually. At appointment it is necessary to consider circadian rhythm of secretion of glucocorticoids: the most part of a dose (2/3) or all dose should be taken in the morning, about 8 o'clock in the morning, and 1/3 - in the evening.
To adults.
In acute conditions and as a substitution therapy, the drug is prescribed in a dose of 20-30 mg per day with a gradual transition to a maintenance daily dose of 5-10 mg. If necessary, the initial dose can be 15-100 mg / day, and the maintenance dose - 5-15 mg / day.
Children.
The initial dose of the drug for children is 1-2 mg / kg / day and is divided into 4-6 receptions, and the maintenance dose is 300-600 μg / kg / day.
The drug is used inside, not liquid and squeezed with a small amount of liquid. Treatment is stopped slowly, gradually reducing the dose.
