Mertenil Indications
- Primary Hypercholesterolemia (IIa type, with the exception of heterozygous familial hypercholesterolaemia) or mixed dyslipidaemia (type IIb), as an adjunct to diet therapy when diet and other non-pharmacological treatments (eg exercise, weight reduction) is insufficient.
- Familial homozygous hypercholesterolemia as an adjunct to diet therapy and other methods of lipid-lowering therapy (eg LDL apheresis) or when such therapy is not effective enough.
- Prevention of cardiovascular disorders. To reduce the risk of serious cardiovascular events in adult patients with an increased risk of atherosclerotic cardiovascular disease (such as age, hypertension, low HDL cholesterol, elevated levels of CRP in the serum, smoking and the presence of a family history of coronary artery disease early development).
- Treatment of Atherosclerosis. In order to slow or delay disease progression in patients who showed lipid-lowering therapy.
Dosage & Administration
Before staring treatment the patient is recommended a standard diet aimed at reducing cholesterol levels, which must adhere to and after drug administration. The dose should be selected according to the purpose of therapy, the effectiveness of treatment and to meet the requirements of today's recommendations.
The medicament is taken orally, at any time of day, regardless of meals, 1 time per day.
Recommended use of the drug in an initial dose of 5 or 10 mg orally once daily 1 to patients who have not previously statins (HMG-CoA reductase) and individuals who received these drugs. When selecting the initial dose should be taken into account the initial concentration of cholesterol in the patient and the risk of other cardiovascular diseases, as well as the potential risk of adverse reactions.
If necessary, after 4 weeks, you can correct the dose.
Given the increase in the frequency of adverse reactions while taking the drug at a dose of 40 mg compared with lower doses, titration of the drug to a maximum dose of 40 mg should be done only for patients with severe hypercholesterolaemia and high risk for cardiovascular disease ( in particular with familial hypercholesterolemia), in which the use of 20 mg did not result in achieving the target level of cholesterol, and which should be under a doctor's supervision. In appointing the 40 mg dose recommended careful medical supervision.
Elderly patients
For patients over the age of 70 years it is recommended to use of the drug at an initial dose of 5 mg (see. SPECIAL INSTRUCTIONS). Another dose correction due to the age of the patient is required.
Contradictions
- Patients with hypersensitivity to rosuvastatin or any of the excipients of the formulation;
- patients with liver disease in the active phase, including those of unknown etiology with a persistent increase in liver transaminases, as well as increased levels of either transaminases in the blood plasma to more than 3 times the upper limit of normal;
- patients with severe renal impairment (creatinine clearance <30 mL / min);
- patients with myopathy;
- patients receiving both cyclosporine;
- pregnancy and breastfeeding;
- women of childbearing age, do not use a reliable means of contraception;
- patients with hereditary galactose intolerance, lactase deficiency, glucose-galactose malabsorption.
