Epiramat Indications
As monotherapy for the treatment of adults and children over the age of 6 years with partial seizures with or without secondarily generalized seizures, and primary generalized tonic-clonic seizures.
As adjunctive therapy for the treatment of adults and children older than 2 years with partial seizures with / without secondarily generalized seizures, or primary generalized tonic-clonic seizures and the treatment of seizures associated with Lennox - Gastaut syndrome.
For the prevention of migraine attacks in adults after careful evaluation of possible alternative treatment. Topiramate is not indicated for acute conditions.
Contradictions
Hypersensitivity to the drug. Prophylaxis of migraine in pregnant women and women of childbearing age that do not use effective methods of contraception.
Dosage & Administration
It is recommended to begin treatment with the lowest dose with subsequent dose titration effective to avoid dose-dependent side effects. The dose level and its increase should be selected in accordance with the treatment effects. Tablets should not crush. Epiramat taken without regard to meals, drinking plenty of fluids.
The drug should be withdrawn gradually to minimize the possibility of increasing the intensity and frequency of attacks. Studies reduced daily doses of 50-100 mg weekly intervals in adults with epilepsy and 25-50 mg in adults treated with topiramate at doses up to 100 mg / day for migraine prophylaxis. In the pediatric studies, the dose of topiramate reduced gradually over 2-8 weeks.
Monitoring the concentration of topiramate in plasma is not necessary for the optimization of drug treatment. In rare cases, to achieve optimal clinical effect of treatment of concomitant therapy with phenytoin, topiramate may require dose adjustment of phenytoin. When adding or canceling of phenytoin or carbamazepine during concomitant therapy with topiramate may require adjustment of dosage.
