Diroton Indications
- Essential hypertension.
- Heart failure (symptomatic treatment).
- Acute myocardial infarction (short-term treatment (6 weeks) hemodynamically stable patients no later than 24 hours after acute myocardial infarction).
- Treatment of primary nephropathy in patients with type II diabetes with hypertension.
Contraindications
- Hypersensitivity to the active substance or excipients of the drug.
- Angioedema in history related to previous treatment with other inhibitors of APF.
- Hereditary or idiopathic angioedema.
- Stenosis of the aortic or mitral valves or hypertrophic cardiomyopathy in violation of hemodynamics.
- Primary hyperaldosteronism.
- Renal artery stenosis (bilateral or unilateral).
- Cardiogenic shock.
- The state of unstable hemodynamics after acute myocardial infarction.
- The second or third trimester of pregnancy.
- Use in patients who are on hemodialysis using vysokoprotochnyh membranes (eg AN 69).
- The level of serum creatinine> 220 mmol / L.
Dosage & Administration
Take once a day, at the same time, regardless of the meal.
Essential hypertension.
The initial dose. The recommended starting dose is usually 10 mg. In patients with very active renin-angiotensin-aldosterone system (especially renovascular hypertension, excessive excretion of sodium chloride and / or dehydration, cardiac decompensation or severe hypertension) may experience an excessive reduction in blood pressure after the first dose. Therefore, early treatment of such patients should be under medical supervision, the initial recommended dose is 2.5 mg * -5. Patients with renal impairment should also reduce the initial dose (see. Table).
Maintenance dose.
The usual effective maintenance dose is 20 mg 1 time a day. If the application intended dose for 2-4 weeks is not achieved the desired therapeutic effect, further dose can be increased. The maximum daily dose should not exceed 80 mg.
