Captopril Indications

- hypertension (hypertensive) disease, symptomatic hypertension;

- chronic heart failure;

- left ventricular dysfunction after myocardial infarction provided clinically stable state;

- diabetic nephropathy in insulin-dependent diabetes mellitus.

Dosage & Administration

Adults. 1 hour before a meal.
Hypertension. the initial dose of captopril is generally 12.5 mg 2 times a day (must be used drugs captopril with a dosage). Note the 1st tolerated doses in the first hour. If the developed hypotension, the patient is transferred to a horizontal position (this reaction to the first dose should not cause the drug withdrawal).

The usual maintenance dose in mild to moderate hypertension (diastolic blood pressure -.. 95-114 mm Hg) is 25 mg (sometimes 12.5 mg 2 times a day). In exchange treatment dose, if necessary, improve after 2-4 weeks, maximum - up to 50 mg 3 times a day.The maximum daily dose is 150 mg. Heart failure. Starting dose - 6.25 mg 3 times per day with gradual (as appropriate) dose escalation to 25 mg 2-3 times a day.

Myocardial infarction. The starting dose is 6.25 mg / day, then the daily dose was increased to 2-3 mg 37,5-75 receive (depending on the tolerability) up to a maximum of - 150 mg / day.

Diabetic nephropathy. Captopril is used in a dose of 75-100 mg / day in 2-3 divided doses. With a total protein clearance of> 500 mg / day, the drug should be taken at a dose of 25 mg 3 times a day.

Impaired renal function. Mild renal impairment (creatinine clearance ≥30 ml / min / 1.73 m2) in patients with diabetic nephropathy is used in a daily dose of 75-100 mg in divided doses.

Contradictions

Hypersensitivity to the drug or to other ACE inhibitors; Angioedema (including a history after ACE inhibitors, hereditary); during pregnancy (may cause severe dysplasia and fetal death) and lactation (discontinued during treatment); aortic stenosis or the presence of other obstacles to the outflow of blood from the left ventricle; bilateral renal artery stenosis or unilateral stenosis of a solitary kidney with progressive azotemia; hyperkalemia; condition after kidney transplantation; Primary aldosteronism.
